What type of documentation do I need for my physical exam?
The Experiential Education Office will send you a physical form that your physician can complete.
If I already completed a physical exam for a previous rotation, will I need to complete another one for future sites?
As long as you have not had a change in your enrollment in the SOP, the physical exam will be valid for your future rotations. The Experiential Education office will ask you to review your physical form each year. If you have not had any health changes, you do not need to complete another physical. If you have had health changes, your physician will need to complete an updated health form. Please note: Sites can request a new physical exam form at any time.
10 Panel Drug Screen
Where can I complete a 10 Panel Drug Screen?

If I already completed a 10 Panel Drug Screen for a previous rotation, will I need to complete another one for future sites?
As long as you have not had a change in your enrollment in the SOP, the 10 Panel Drug Screen will be valid for your future rotations. Please note: Sites can request a drug screen at any time.
Which option should I choose for my 10 Panel Drug Screen?
Before scheduling your drug screen, confirm with the facility that your lab results will be assigned to a Medical Review Officer (MRO). There may be facilities that do not assign an MRO for the rapid result option.

How do I know if my site has other requirements in addition to the the SOP health requirements?
Site requirements are listed on the clerkship webpage under Student Pre-Activity for each site. The Experiential Education Office Staff will also inform you of additional requirements prior to each block.
How do I submit my documents?
The Experiential Education Office will provide instructions on how to submit your required documents.
Background Check
My site is requesting a Caregiver Background Check. How do I complete this requirement?
Go to Wisconsin Caregiver Background Check and complete the on-line form.
For the background request type, select “Caregiver”. There is a cost associated. Refer to website for current costs.
