Preceptor and Site Evaluations
DPh-1, -2, and -3 students are required to submit online evaluations of preceptors and sites for each rotation (community, institutional, and elective). Evaluations are required from the School of Pharmacy’s perspective to ensure the quality of our preceptors and sites. Additionally, preceptors value constructive feedback given by students and use it to improve their teaching and sites for future students. For these reasons, students need to complete two evaluations (preceptor and site) after finishing their required hours for each rotation. The evaluations are located at the pharmacy clerkship website. These course evaluations should be completed prior to the last day of the overall “parent” block for 2nd and 3rd year students (e.g. if you are assigned to block 2-3, these evaluations are due on the final day of the 2nd block (or the final day of block 2-6), and within 7 days of the end of the assigned block for 1st year students. Students not completing these rotation evaluations will receive a grade reduction as described below.

Note: Two evaluations are required (preceptor and site) for each experiential rotation.
Students must finalize the evaluation for it to be considered complete.
Student Performance Evaluation (SPE)
Clinical instructors precepting DPh-1 and DPh-2 students will use a shortened Student Performance Evaluation to evaluate IPPE students on rotations. This tool includes the following grading sections: Cultural Sensitivity, Verbal Communication, Self-Awareness, and Professionalism.
Clinical instructors precepting DPh-3 students will use a slightly longer Student Performance Evaluation to evaluate IPPE students on rotations. This evaluation includes the following grading components: Knowledge, Pharmacists’ Patient Care Process, Medication Use Systems, Health and Wellness, Cultural Sensitivity, Verbal Communication, Self-Awareness, and Professionalism.
