Grading Summary
A single grade will be received at the end of spring semester for the 4 rotations completed in 728-625 and 728-626. Students who are making progress in the course will receive a grade of “P” at the end of the fall semester. The final grade for the course will be composed of a summary of grades earned in each of the rotations. (Please Note: Even though there is one final grade for the combined fall and spring semesters, it is necessary that ALL students register for 728-625 in the fall semester and 728-626 in the spring semester to permit posting of a final grade).

Depending on the activity, if a percentage is entered directly into Canvas, the point value will be converted to a percentage grade rounded to the nearest 0.1%. The final grade will be calculated from the components listed above, each rounded to the nearest 0.1%. The final grade will be rounded to the nearest 0.1% (e.g. 92.93% = 92.9% = AB; 92.97 = 93.0 = A). Once the final grade is calculated to the nearest 0.1%, no further rounding will occur.
Grading Rubric
For each rotation, the clinical instructor will observe the student complete activities on the rotation checklist, evaluate the student’s performance using tools available at the clerkship website, and provide student feedback. The checklist describing activities for each type of rotation can be found on the Experiential Education Manuals website as well as from the Appendices of this manual. At the end of each rotation, the clinical instructor will go the clerkship website to acknowledge that all elements of the checklist have been completed, complete the 13-point student performance evaluation and verify that the student has completed the required number of hours for the rotation by checking the appropriate box. Clinical instructors will also be asked to acknowledge that the student contacted them at least 2 weeks prior to the rotation and provided student profile information.
Community Pharmacy [630] Rotation: 22%
- Completion of hours and Community Pharmacy Rotation Checklist (12%)
- Student is required to upload checklist to Canvas drop box by deadline.
- Clinical Instructor is responsible for verifying hours and activities completed on the grading record.
- Student Performance Evaluation (10%)
- Clinical instructor responsible for completing student performance evaluation on Clerkship Website. The score on the SPE will make up 10% of the final grade.
Institutional [635] Care Rotation: 27%
- Completion of hours and Institutional Care Rotation Checklist (17%)
- Student is required to upload checklist to Canvas drop box by deadline.
- Clinical Instructor is responsible for verifying hours and activities completed on the grading record.
- Student Performance Evaluation (10%)
- Clinical instructor responsible for completing student performance evaluation on Clerkship Website. The score on the SPE will make up 10% of the final grade.
For Each Elective [645] Rotation: 13% (x 2 Rotations = 26%)

- Student Performance Evaluation (5%)
- Clinical instructor responsible for completing student performance evaluation on Clerkship Website. The score on the SPE will make up 5% of the final grade.
Clinical Inquiry: 20%
- Workshop attendance and activities will make up 5% of the clinical inquiry score and completion of final clinical inquiry will make up 15% of the score.

SMART Goals and Reflections: 5%
- Students will be required to submit SMART goals to Canvas by specified deadline.
- Students will be required to submit 4 reflections (1 for each rotation) over the course of the academic year. Each reflection is due by the specified deadline for the corresponding block.
Additional Grading Information
Contact Preceptor Prior to Rotation*. Students who do not contact their preceptor two weeks in advance during one rotation will receive a half-letter grade deduction (e.g., A to AB). Students who fail to contact more than one preceptor two weeks in advance will receive a full-letter grade deduction (e.g., A to B).
Health Policy*. Students not in compliance with School Health Policies documentation will receive a half-letter final grade deduction (e.g., A to AB) as outlined by the Experiential Education Policies Manual.
Missed Rotation. All experiential hours must be completed in the assigned block. If hours are completed either prior-to or after the assigned block dates without course coordinator approval OR an experiential rotation has to be rescheduled to another block due to circumstances that were in the student’s control (e.g., student failed to contact preceptor, student failed to show up to rotation, student did not complete necessary site trainings on time), then student will receive a full-letter grade deduction (e.g., A to B).
* If rotation is missed due to not contacting preceptor in advance or due to health policy, deductions will not exceed a full-letter grade. See table below for example scenarios.

Rotation Checklists. Students are expected to upload a completed checklist (signed by preceptor and student) to the appropriate drop box in Canvas by 11:59pm on the last day of the block. Students who fail to upload their checklist by the deadline will receive a 2% grade deduction for each checklist. For example, if a student submits a checklist late for the community rotation and for the institutional rotation, then the final grade will be reduced by 4% (e.g. 95% to 91%). Incomplete checklists are subject to review by the course coordinator.
Site & Preceptor Evaluations. Students are expected to complete both site and preceptor evaluations for each rotation. These evaluations are due by 11:59pm on the last day of the block. Students who do not complete these evaluations prior to the deadline for one rotation will receive a half-letter grade deduction (e.g., A to AB). Students who fail to complete these evaluations prior to the deadline for more than one rotation will receive a full grade deduction (e.g., A to B).
