Evaluation Overview
Rotation Assignments/Activities. (50%) Students MUST demonstrate proficiency in the specified institutional activities. Activities may be assigned a score of 0 to 12 based on the associated assessment scale or an assessment of yes/no. Please see individual assignments/activities for the evaluation method. Preceptors will record a score on the grading record. Calculation of an individual score’s contribution to the overall grade for the rotation will be performed by the School of Pharmacy.
Students MUST earn a final score of 6 or better on each health-system pharmacy practice activity that requires a number score. Failure to earn a score of 6 on an activity requiring a score will result in repeating the activity until a score of 6 or more is achieved. In these cases, the maximum score earned for the activity is a “6”. The following activities are exempt from being repeated: DUE/MUE, Therapeutic Protocol, and Drug use guideline. In these cases, the first score earned is the score reported.
